Diabetes Care Program

More than 100 million Americans have diabetes or prediabetes. Left unchecked, diabetes can take a toll on nearly every organ in your body, including: heart, eyes, kidneys, nerves, GI tract, gums and teeth. Diabetes has long been associated with a shorter-than-average life span and a lower quality of life. 

However, this does not have to be the case. Diabetes and prediabetes is a manageable and even reversible* disease. In fact, many people who have lived with undiagnosed diabetes, find their quality of life actually improves after a diagnosis. 

*The term reversal is used when people can go off medication but still must engage in a lifestyle program in order to stay off, according to Ann Albright, PhD, RD, the director of diabetes translation at the CDC.

Don’t let a diabetes diagnosis prevent you from living the life you desire.   

Our program is different:  We are not here to tell you what to do.  We do not preach health practices to you.  We are your partner.  We are here to listen to what you want and help guide and support you.  We provide resources and accountability in a non-threatening, positive environment. 

Once enrolled on our program you will work one on one with a nurse-coach.  Together you will co-create a plan that works for you.  You will define your personal health vision and set your personal health goals.  Along the way, your nurse-coach will be available to answer questions, share information, help you identify and overcome barriers, stay connected and provide support to help you stay on track. 

You have the ability to work with your nurse-coach as often as you wish at times that are convenient for you. You connect via phone, secure messaging or video chat.  There are no boundaries in this program.   We meet you where you are and work side by side to help you reach your goals.
